Idaho Birth Certificate
When giving birth in an Idaho hospital or birth center, getting the birth certificate for a newborn is taken care of during the hospital stay by the nursing staff. In home births by midwives, the midwife submits the necessary paperwork on behalf of the parents. To get an Idaho birth certificate for a home birth that doesn't involve a midwife, new parents must show proof of pregnancy and Idaho residency.

Obtain proof of pregnancy from your doctor. During pregnancy, you will have many opportunities to get proof of pregnancy through ultrasounds, tests and other doctors office visits; the document must show the mother's name and the doctor's name to be valid. Save these documents for future use.

Contact the Idaho Bureau of Vital Records and Health Statistics after the baby is born and request a Home Birth Worksheet. The person in charge of Birth Registration will mail a worksheet to your home with the instructions for applying for a birth certificate for a child born at home.

Mail the completed worksheet along with required documents. The Idaho Bureau of Vital Records and Health Statistics requires parents to show proof of pregnancy and residency. Utility bills that show the parent's name and address fulfill the requirements of proof of residency.

Sign and return the completed birth certificate. Once the vital records office receives the completed worksheet, they will complete a birth certificate and mail it back for signature of the child's parent(s). Sign the certificate and return it along with the proof of residency and pregnancy. If you do not have the required documents, the vital records office allows you to send them later. However, you must return the signed birth certificate within 15 days.

Receive the official birth certificate. After the vital records office receives the signed birth certificate and all the required documents, they will mail a certified copy of the child's birth certificate.
